The InsureGA initiative has been selected by the U.S. Department of Health and Human Services and Centers for Medicare and Medicaid Services to serve as a Navigator entity in the State of Georgia. Our work is supported by Funding Opportunity Number CA-NAV-16-001. As such, we are authorized to collect personally identifiable information under section 1311(i)(3) of the Affordable Care Act, 45 CFR 155.210(e), the Cooperative Agreement to Support Navigators in Federally-Facilitated and State Partnership Marketplaces Funding Opportunity Announcement (“Navigator FOA”), and 45 CFR 155.215(a)(1)(iii).
The information provided about you will be used to perform the following duties: providing information that assists consumers with submitting the eligibility application; clarifying the distinctions among health coverage options, including Qualified Health Plans; helping consumers make informed decisions during the health coverage selection process; facilitating the selection of a Qualified Health Plan; provide referrals to any applicable office of health insurance consumer assistance for any enrollee with a grievance, complaint, or question regarding their health plan, coverage, or a determination under such plan or coverage; and to provide information to consumers about the full range of Qualified Health Plan options and insurance affordability programs for which they are eligible.
While providing the requested information is voluntary, failing to provide it may delay or prevent your ability to obtain health coverage through the Marketplace, advanced payment of the premium tax credits, cost sharing reductions, or an exemption from the shared responsibility payment.
